The goal of Montvale Orthodontics is to give you the beautiful, healthy smile you’ve always wanted. Whether you are a concerned parent looking into orthodontic care for your children or a prospective patient who is ready to improve how you feel about your smile, we understand that this is a big decision. While we use the latest technology in orthodontic care, we realize that being comfortable and confident with our practice is a bigger concern for the typical patient. That’s why we’ve designed our process to help you feel comfortable with Dr. Don Oh and his staff.
During your first visit to our office, we will learn about your orthodontic needs. We make sure to obtain important background information, like your medical history, and give you time to get to know Dr. Oh. It is recommended to take diagnostic records so that Dr. Oh can thoroughly review them and provide you with choices of treatment plans. Dr. Oh will meet with you and go over every single detail of the findings, including treatment plans, choices of different appliances, and timelines, so you have the best idea of what to expect going forward.
New Patient Forms
You can save some time on your first visit by printing out and completing the patient forms in advance of your appointment.
On the first visit, please bring the following information:
- A valid form of ID.
- Orthodontics insurance card, if your family is covered and you are paying through insurance.
- Any x-rays taken by a previous dentist. X-rays can also be forwarded to our office from your previous dentist upon your request.
Insurance and Financing
We believe quality orthodontic care should be affordable and accessible to everyone. Because oral health is so important, we do everything we can to remove financial barriers and help the greatest number of patients possible. Our office is in-network with many major providers and accept almost all PPO plans, New Jersey FamilyCare/Medicaid, and New York Medicaid.
